3. Words of Endearment and Other Special Lettering - How much and where?

4. Special Designs, Emblems (fraternal, civic or other type of emblems) and/or Ceramic Photos - Who was this person you loved and how do you want to think of or remember them?

Standard Pricing: includes any of our Standard designs, 2 emblems, 4 words of endearment, Full name, birth and death dates without a blank 2nd scroll date.

If you are going to order a bronze on granite grave marker, you must have the cemetery approve this form for acceptance of your grave marker before delivery can take place.
